By the first week of April every quality director should have a complete read on the Q1 quality story. Six elements separate a credible Q1 dashboard from a generic one. Together they make the difference between a quality function that intervenes in time and one that explains in November.
1. MIPS composite trajectory
Year-to-date composite score by category. The point isn't the absolute number — Q1 is too early for the absolute number to matter. The point is the trajectory: are we tracking toward 75+, or are we behind?
2. Quality measure-by-measure heatmap
Each selected MIPS quality measure with current rate, benchmark, and YoY delta. Red flags:
- Measures dropped from Q4 last year by >5 percentage points.
- Measures with denominator volume <30% of expected for the time of year.
- Measures where one provider is dragging the rate down significantly.
3. Promoting Interoperability category status
PI floors at zero — a single missed measure tanks the entire category. Q1 dashboard must include:
- SAFER Guides attestation status.
- Direct Messaging count vs. annual target trajectory.
- Public health registry transmission success.
- e-Prescribing percentages.
4. Improvement Activity (IA) attestation status
IA points are easy to forget about until November. Q1 is the right time to confirm:
- Which IAs the practice has committed to.
- Documentation evidence in place for each.
- Total IA points committed (target: 40 for non-small practice).
5. Cost category leading indicators
Cost is calculated by CMS, not submitted. Q1 dashboard should show:
- MSPB trend year-over-year.
- Outlier cohorts driving total cost.
- Service-line specific cost metrics where applicable.
6. Workflow gap surfaces
For each underperforming measure, the dashboard should drill into the patient list. A 60% diabetes A1C poor control rate is just a number until you see the 47 specific patients driving it. Then it's an outreach plan.
