UDS Reporting (Uniform Data System)

The Uniform Data System (UDS) is HRSA's mandatory annual reporting system for Federally Qualified Health Centers, collecting standardized clinical quality, financial, and operational data that determines grant compliance, public performance rankings, and FQHC designation renewal.

What is UDS Reporting?

The Uniform Data System is HRSA's mechanism for collecting standardized performance data from all FQHCs (Federally Qualified Health Centers) and FQHC Look-Alikes annually. UDS data populates the HRSA Health Center Locator and is publicly displayed for all health centers. The UDS report is organized into tables: Table 3A captures patients by payer (Medicare, Medicaid, CHIP, other public, private, uninsured); Table 3B documents patients by age and sex; Table 4 covers visit counts by service type; Table 5 tracks staffing and productivity; Table 6A records selected diagnoses and services rendered; Table 6B is the most critical — it reports 24 clinical quality measures including HbA1c control (A1C), blood pressure control, colorectal cancer screening, depression screening, and tobacco cessation counseling. Table 7 captures health center staffing costs, and Table 8A contains financial data. The UDS+ supplement, launched in 2023, uses FHIR-based data collection to capture more granular race, ethnicity, and social determinants of health (SDOH) data. UDS requires race and ethnicity stratification of quality measures, enabling health equity analysis. Failure to submit UDS data or falling below HRSA quality thresholds can affect Section 330 grant renewal.

Why It Matters for Healthcare Analytics

For FQHCs, UDS is not optional — it is the primary accountability mechanism for Section 330 funding. Understanding performance on each of the 24 Table 6B clinical quality measures, stratified by race/ethnicity and payer, identifies both grant compliance risk and health equity gaps. HRSA compares FQHC performance nationally, making benchmarking against peer health centers essential.
